Policies:

Payment due at the time of enrollment. For all services payment is due at the time treatment. CREDIT CARDS/CARE CREDIT accepted.

48 hour notice is required to change an appointment time reserved for you. Cannot be used with any other plans, dental insurance, or discounts. Plan transferable and nonrefundable.
